ICYMI: From the Division of Quality Assurance: E-Background Information Disclosure webform – F-82064

New Electronic Background Information Disclosure (eBID) Webform

The Division of Quality Assurance (DQA), Office of Caregiver Quality (OCQ) announced the release of its new E-Background Information Disclosure (eBID) webform, F-82064.

A link to the webform may be sent to applicants, staff, or contractors when entities complete background checks required by DHS 12. The completed form can be emailed to the entity and held on file electronically to document compliance.

Please note that the existing downloadable PDF form will remain available in the DHS Forms Library, but will be phased-out on March 1, 2026.
