Required Direct Care Workforce Funding Survey Due April 30
If you are a provider who receives Direct Care Workforce Funding, you must annually complete a simple survey from the Department of Health Services (DHS) to continue participation in the initiative. This year’s survey is now available – please see the important information below from DHS.
The 2026 Direct Care Workforce Survey is available for providers to complete. Responses are due Thursday, April 30. Providers should complete this survey based on all direct care workforce payments received since June 2025. These payments include:
- 2024 Payment 2 distributed in June 2025
- 2025 Payment 1 distributed in December 2025
The survey is consistent with last year’s format which was streamlined to collect high level information from providers regarding the amount received from these payments and how this amount was spent. Control numbers do not need to be entered. Completion of the survey is a requirement of participation in the DCW Funding Initiative – non completion will jeopardize future DCW payments. The results will only be used for reporting purposes on the effectiveness of these payments and will not be used as validation for future payments.
