New Resource! Nursing Home ADE Algorithm
Superior Health Quality Alliance (Superior Health) created an algorithm to follow when a resident returns to the nursing home after hospitalization or an emergency room visit. If the resident is on one or more high-risk medications, they may be assigned a diagnosis code that indicates an adverse drug event (ADE). The diagnosis may be appropriate for the resident and the event that lead to the hospital visit or that diagnosis should be discontinued. In either case, documentation to support the event must reflect either scenario. This algorithm will guide you through a step-by-step process to ensure continuity of care.
