Important Home and Community-Based Services Provider Enrollment Information – Applies to Family Care Participating Facilities

As announced earlier this year, all providers of home and community-based services under an adult long-term care (LTC) waiver program must enroll with Wisconsin Medicaid through the ForwardHealth Portal by January 1, 2026.

As of today, 3,500 providers have not started an enrollment application. That means they won’t get paid to deliver Medicaid services in 2026. Please see the resources and information available below to complete your mandatory enrollment within the portal before January 1, 2026.

DHS has updated the following toolkits to include language emphasizing that time is running out to get provider enrollment applications processed in time to avoid payment delays and service disruptions in 2026. They have also added a few new resources, like digital display graphics and an informational video. Please check them out and begin using the refreshed materials right away.

Reminders

As of August, all adult LTC waiver services providers can enroll on the ForwardHealth Portal.

  • Supportive home care agencies can submit provider enrollment applications.
  • Supportive home care agencies with an electronic visit verification (EVV)-only provider Medicaid ID can convert their EVV-only enrollment to full Medicaid enrollment (revalidate).

This requirement does not affect individual self-directed support or participant-hired workers.
