Updated toolkits for Adult Long-Term Care Provider Enrollment

As you know, all providers of home and community-based services under an adult long-term care (LTC) waiver program must enroll with Wisconsin Medicaid through the ForwardHealth Portal by January 1, 2026.

We need your help—as of today, only 1,995 providers have started an enrollment application or finished enrolling. That means a majority of providers have yet to start an enrollment application.

In March, we shared toolkits for managed care organizations (MCOs), fiscal employer agents (FEAs), IRIS consultant agencies (ICAs), and IRIS participants. These toolkits have a wealth of ready-to-use materials for these key partners to share with their providers and help them have conversations about the new enrollment requirement. These toolkits are now updated with new and improved items:

What’s new or updated

  • *New* Enrollment Progress Tracker that shares statewide progress on enrollment
  • Updated provider flyer, email, short article, and FEA support line talking points with stronger language explaining the consequences for not enrolling
  • Updated short banner message emphasizing getting paid in 2026
  • *New* long banner message for applications with more space
  • Updated short IRIS participant article, ICA talking points, IRIS tip sheet, and IRIS participant talking points to emphasize that their providers and vendors won’t be able to deliver their services in 2026 unless they enroll

Reminders

  • This requirement does not affect individual self-directed support or participant-hired workers.
  • Currently, supportive home care organizations or providers with a unique EVV provider agency ID cannot apply because their provider type is not yet available in the application. We will let you know when that changes in August.
