Nursing Home Case Mix Index Rosters Available
Case mix index (CMI) resident rosters for the picture quarter ending December 31, 2025 (based on data available as of July 11, 2026) are now available on the ForwardHealth Portal. These rosters contain Patient Driven Payment Model (PDPM) case mix index values for residents.
Annualized roster data available as of June 30, 2026, will be used to calculate fee-for-service daily rate factors (all-resident CMI and the behavioral and cognitive impairment [BEHCI]).

Facility-specific interim or final nursing home rates by acuity category (typically used by managed care organizations) are updated monthly on the Myers and Stauffer Wisconsin Department of Health Services webpage.
