Oak Park Place is hiring for an RN Admissions Director for our beautiful Madison Nakoma (West Side) location. This is an exciting opportunity to join our growing organization in offering exceptional care to seniors in a warm and caring environment. As a smaller company, we cater to our resident’s needs, and take pride in providing an employee friendly work environment. We are looking for a nurse to oversee our admissions process for our Skilled Nursing side and also do the assessments for our Assisted Living/Memory Care side.

What you’ll be doing as the Admissions Nurse:

You will oversee the admission systems, controls and alliances with healthcare networks
Work with Community Liaison and hospital discharge planners
Working with fellow colleagues as a point of contact to offer support and assistance to ensure the admission process runs smoothly
Preparing paperwork, rooms, and staff for new admissions in both the long-term care,memory care, and our outstanding rehab unit
Work closely with the Licensed Nursing Home Administrator as well as center staff to ensure a smooth, seamless transition into our center
Consult and coordinate with healthcare team members to assess, plan, implement, and evaluate resident care plans
Assess,plan, and evaluate care delivery systems, while monitoring resident outcomes
Participate in resolution of resident, family, staff compliments, complaints and concerns
Maintain an “on call” status for emergencies on a rotating basis
Complete and document Weekly Assessment tools
Utilize established standards/criteria to oversee resident care services
Potentially help our other campuses with their admissions process and systems
Qualifications:

WI RN License
Admissions experience preferred but willing to train the right individual
Supervisory experience in a skilled nursing facility
Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care
