Maple Ridge Care Center is looking for a MDS Coordinator that is an RN, who is compassionate about caring for our senior population in the nursing home setting. Our team of nurses and nursing assistants strive to provide best practice and resident centered care.
Job Summary
In conjunction with Director of Nursing the MDS Coordinator is responsible for overall completion of each resident’s MDS, RAI and assessment.
Qualifications Required
· Current state licensure as a Registered Nurse, which must be in good standing
· Knowledge of current laws and regulations regarding skilled nursing and long term care
· Must be literate and possess excellent written and verbal communication skills

Essential Functions of the Job, Departmental expectations
· Utilizes pertinent documentation obtained from the medical record to accurately complete MDS/RAI and assessments and transfer the correct information to the MDS document.
· Observant for significant changes in resident’s condition for the purpose of the MDS.
· Maintains and monitors individual MDS.RAI to maintain an optimal reimbursement level.
· Monitors, organizes and assists nursing staff in setting up a processes whereby the nursing staff completes the observations according to the MDS schedule
· Transmits MDS’s that have been completed into the state data base weekly and as needed
· Completes, prints and binders the 802 and 672 forms and have them available for state weekly and as needed.
· Completes all necessary and required forms/documentation in a timely manner
