Importantly, in very recent meetings with Division of Quality Assurance (DQA) leadership, they highlighted three areas of particular concern:

F 684:  Failure to promptly recognize a change in a resident’s condition and then taking proactive action in response;

F 686:  Pressure Injuries

F 689:  Preventable Falls with Serious Injury
