Adult Long-Term Care Provider Enrollment Now Open!

Starting September 16, providers of home and community-based services (HCBS) for adults can enroll with Wisconsin Medicaid through the ForwardHealth Portal. All adult long-term care (LTC) waiver services providers must use this new provider enrollment process by January 1, 2026, to continue delivering services to Medicaid members.

This applies to providers of:

  • Family Care
  • Family Care Partnership
  • Program of All-Inclusive Care for the Elderly (PACE)
  • IRIS (Include, Respect, I Self-Direct)

Note: Supportive home care (SHC) organizations should not enroll yet. Your provider type is not available in the application. DHS will let you know when you can enroll, likely in mid-2025.

Getting help

Providers are encouraged to attend a live virtual training on Zoom to help them understand how to enroll. There are upcoming live training sessions for long term care providers listed in the linked documents below.

Trainings will cover the enrollment timeline and demonstrate how to enroll or how to add waiver services and waiver programs to an enrollment. The New Provider Enrollment System for Adult Long-Term Care page on the Portal has a Live Training Schedule in the Quick Links menu.

Partners and providers should make sure they are signed up for the Adult LTC Waiver Provider email subscription list to receive training registration links by email.
