The 2025 3rd Quarter Quality Improvement (QI) Variables submission period is now OPEN and will remain open through October 15th. Please submit your data as soon as possible! Instructions are included below.

How to Submit the QI Variables:

  • Click the eQuality Tab
  • Click the QI Variables Tab
  • Click QI Variables Form gray button. (The photo below shows where to locate the form)
